Welcome to our office and thank you for selecting us to help with your dental care. We pride ourselves on making dentistry a

pleasant experience. We are committed to providing you with the highest quality of oral health care in the most gentle, efficient manner possible.

Our primary goal is to keep your teeth a lifetime. We can reach this goal only by keeping you well informed at all times. This letter is to let you know what we will accomplish on your first visit to our office.

During your first visit, you can expect:

• an introduction to our office and staff
• a thorough examination of your mouth, teeth, and gums
• any necessary x-rays to be taken
• complete oral cancer screening exam
• an evaluation for any decayed, broken, or worn teeth
• a thorough screening for any gum or periodontal disease
• if needed, study models of your mouth

From this examination, we will list and discuss all treatment needed to restore your mouth to optimal dental health. We will then plan the appointments needed to reach this goal. Keep in mind that the purpose of this visit is not for a cleaning. It is a chance for us to get to know you and to completely evaluate your dental condition.

We ask that you complete both a health history form and patient registration form before your first visit.  If possible, please send them back to us at least 2 weeks prior to your appointment, or bring them with you for your appointment. If you have dental insurance, include your insurance form completed and signed. We customarily expect at least partial payment for services, depending on the estimated amount your insurance company will cover.
